
Registered Nurse – Transfer Center Specialist

This is a fully remote position, open to applicants in Ohio.
• Coordinate the logistics of patient transfers by utilizing workflows identified by clients to assist with patient acceptance and bed placement.
• Assess the appropriate level of care, accepting provider, consultation, and access from referral sources.
• Collaborate with clinicians to convey accurate clinical presentations, facilitate timely access and bed assignments, and document patient information effectively.
• Input precise and timely data into EHR, telephony, and office systems while ensuring the protection of PHI.
• Recommend patient outcomes based on clinical information, individual preferences, and the capabilities of transferring and receiving facilities.
• Serve as a liaison among patients, physicians, hospital personnel, client facilities, and external agencies.
• Oversee team compliance and performance while supporting process changes for regulatory and quality initiatives.
• Utilize knowledge of EMTALA, Ethics and Compliance, quality initiatives, and HIPAA to report risks and operational inefficiencies.
• Collaborate with healthcare providers and teams to ensure timely acceptance and access to the appropriate level of care.
• Engage in process improvement, professional development, peer review, data collection, and shared leadership initiatives.
• Execute additional tasks and analyses as assigned.
• Active Registered Nurse (RN) licensure — Required.
• Active multistate/compact RN licensure — Required if applicable based on state of residence.
• Associate Degree in Nursing or diploma (ADN) — Required.
• Minimum of 1 year of acute care experience — Required.
• Maintain active RN licensure in the states where transfer center services are offered.
• Designated workspace with a locked door that complies with HIPAA regulations.
• A secondary work location that meets HIPAA regulations is encouraged for power outage contingencies.
• Minimum internet speed of 100 Mbps download and 20 Mbps upload at both primary and secondary work locations.
• Proficiency in EHR data entry, telephony systems, and office management systems.
• Familiarity with EMTALA, Ethics and Compliance, quality initiatives, and HIPAA regulations.
• Competency in Microsoft Office applications, including Outlook, MS Teams, Excel, and Word.
• Capability to review and interpret patient transfer case information.
• Ability to escalate and inform relevant parties regarding changes in patient status.
• Skill in documenting tasks and conversations related to patient transfers.
• Strong critical thinking and clinical decision-making abilities.
• Excellent oral and written communication skills.
• BSN — Preferred.
• 3+ years of acute care experience — Preferred.
• Experience in healthcare contact centers — Preferred.
• Familiarity with Epic electronic health record — Preferred.
• Experience with Genesys contact center — Preferred.
