Registered Nurse, Telehealth Medicare Annual Wellness Visit Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct Medicare Annual Wellness Visits through telehealth in alignment with CMS guidelines, ensuring the completion of all necessary assessments and screenings.

• Review and update patient medical histories virtually, including medication lists, immunization records, and family backgrounds.

• Evaluate patients’ current health status, including vital signs if available, cognitive function, and risk factors for chronic diseases, utilizing remote monitoring tools when applicable.

• Educate patients on preventive measures, lifestyle adjustments, and available community resources to enhance overall health and wellness through virtual consultations.

• Create and revise personalized prevention plans tailored to individual patient needs and physician recommendations.

• Work in collaboration with the supervising physician to ensure precise documentation and follow-up on any identified health issues using electronic health record systems.

• Identify any HCC codes that require recapture reporting to CMS.

• Maintain thorough and accurate patient records in compliance with HIPAA and organizational standards, particularly within a remote setting.

• Facilitate virtual referrals to appropriate healthcare services and specialists as necessary.

• Monitor patient progress and adherence to personalized prevention plans, providing support and making adjustments as required through telehealth follow-ups.

• Stay updated on Medicare guidelines, telehealth best practices, preventive care methods, and chronic disease management strategies.

• Engage in virtual team meetings, training sessions, and continuing education to sustain and enhance clinical skills.

⛳️ Requirements

• Must be fully bilingual in Spanish and English.

• Must reside in GA.

• Current and unrestricted Registered Nurse (RN) license in the state of GA.

• Availability for PRN work Monday through Friday, from 8:00 am to 5:00 pm, approximately 20 hours per week.

• At least 3 years of patient care nursing experience, preferably in a primary care, preventive care, or telehealth environment.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly preferred.

• Strong understanding of Medicare guidelines, telehealth practices, and preventive care strategies is preferred.

• Excellent interpersonal and communication abilities, capable of effectively educating and engaging patients in a virtual setting.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Ability to work independently and manage time efficiently in a remote work environment.

• Strong organizational skills and keen attention to detail.
