This is a fully remote position, open to applicants in Maryland.
• This role operates independently while also collaborating with all members of the healthcare team to ensure high-quality, cost-effective care, reducing fragmentation within the healthcare delivery system for patients with ESRD and CKD.
• This position is responsible for coordinating care among the patient, their family, physicians, providers, care teams, the community, and the Regional Operations Manager.

• An A.D.N degree from an accredited nursing school is required; a B.S.N is preferred; a three-year diploma from an accredited nursing program may be accepted in lieu of a nursing degree.
• A current Registered Nurse (RN) license in the state of practice is mandatory.
• Continuing education credits must be maintained as required by the state of practice.
• A minimum of five (5) years of clinical nursing experience is required.
• A minimum of three (3) years of renal nursing experience is preferred.
• Strong preference for candidates with case management experience, showcasing the ability to assess patient needs, create and manage personalized care plans, coordinate services across interdisciplinary teams, and guide patients through complex healthcare transitions.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• Proven knowledge and understanding of data management related to clinical, financial, and patient satisfaction outcomes is required.
• Demonstrated experience and effectiveness in a change agent role is necessary.
• Comprehensive understanding of Continuous Quality Improvement (CQI) techniques is required.
• Previous experience in healthcare performance coaching is mandatory.
• A current CPR certification is required.
• Ability to adapt personal practice patterns to new/electronic processes and heightened productivity expectations related to Capella implementation is essential.
• A current driver’s license in the state of employment with a positive driving record is necessary, along with the ability to meet insurance coverage requirements.
• Basic computer skills and proficiency in MS Word and Outlook are required.
• Functional proficiency with DaVita-specific clinical software programs, including Capella, is required within 90 days of employment.
• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
