This is a fully remote position, open to applicants in Texas.
• Deliver compassionate and proficient nursing care to patients on a full-time basis in the comfort of their residences.
• Conduct thorough assessments of patients' physical, emotional, and social needs to create tailored care plans.
• Work collaboratively with patients, their families, and healthcare teams to set goals and interventions aimed at addressing patients' health issues and enhancing self-management.
• Regularly evaluate patients' conditions, monitor vital signs, and assess the effectiveness of interventions, modifying care plans as needed.
• Educate patients and their families on managing diseases, administering medications, and utilizing self-care techniques.
• Provide skilled nursing services, including wound care, medication management, IV therapy, catheter care, and other prescribed treatments.
• Administer medications and treatments accurately and safely, adhering to established protocols and guidelines.
• Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy.
• Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to relieve distress.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and execute comprehensive care plans.
• Coordinate with the interdisciplinary team to guarantee continuity of care and effective communication.
• Maintain regular communication with patients, families, and caregivers to provide updates on the patient's condition, progress, and any modifications to the care plan.
• Oversee and supervise home health aides and other support staff involved in patient care.
• Keep accurate, timely, and comprehensive documentation of assessments, care provided, and patient outcomes utilizing Nestmed AI charting tools and the Kinnser EHR system.
• Ensure adherence to all regulatory requirements, agency policies, and standards of care.
• Engage in quality improvement initiatives, including case conferences, chart reviews, and performance improvement projects.
• Valid and unrestricted Registered Nurse (RN) license in the State of Texas.
• At least one year of nursing experience in home health or a related setting.
• Proficient in using EHR systems (Kinnser/WellSky preferred), Nestmed, or other AI charting tools, as well as applications like Slack.
• Familiarity with home health regulations, Medicare/Medicaid guidelines, and OASIS documentation.
• Strong clinical assessment and critical thinking abilities to effectively address patients' complex needs.
• Excellent communication and interpersonal skills to build rapport with patients, families, and healthcare professionals.
• Capability to work independently, prioritize tasks, and manage time effectively in a dynamic home health setting.
• Willingness to travel to patients' homes within the designated service area and possess reliable transportation.
• BLS (Basic Life Support) certification.
