Registered Nurse – Case Manager

This is a fully remote position, open to applicants in Illinois.

📋 Description

• This role is tasked with implementing medical management and health education initiatives for clients enrolled in government health care programs.

• Responsibilities include collecting, analyzing, and delivering data for regulatory documentation.

• This position will act as a representative of the company to its members.

⛳️ Requirements

• A Registered Nurse (RN) with three years of direct clinical care experience in a clinical environment, or a Licensed Professional Counselor (LPC), or a Licensed Master Social Worker (LMSW), which includes two years of clinical practice necessary for obtaining their LPC or LMSW license.

• A current, valid, and unrestricted license in the state where the position is based (or through reciprocity).

• For compact licensees relocating permanently to the state of operations, it is required to secure an active, unrestricted RN license in the state of operations within 90 days of employment.

• Additionally, three years of experience in wellness or managed care, addressing clinical concerns with members and physicians.

• Familiarity with the health and wellness market and trends among employers.

• Strong verbal and written communication abilities, including the capacity to discuss medical needs with members and to engage with internal staff, management, external vendors, and community resources.

• Experience in analyzing medical data.

• Willingness and ability to travel within the assigned territory.

• Proficient in PC applications including Word, Excel, PowerPoint, database management, and web-based tools.

• Three years of clinical experience.

• Experience in patient education.

• Experience in Condition Management.

• Bilingual proficiency in English and Spanish.

• Experience in Transition of Care.

• Background in managing complex or catastrophic cases.

• Certification in Case Management, Training, Project Management, or a nationally recognized health care certification.
