Registered Nurse, Care Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Collaborate closely with individuals facing intricate medical, behavioral, and social challenges.

• Perform thorough assessments to evaluate medical, behavioral, functional, and social factors influencing health.

• Create personalized care plans tailored to members' distinct needs, objectives, and preferences.

• Utilize active listening and motivational interviewing methods to assist members in comprehending their health.

• Support members in navigating complex healthcare choices and ensure they have access to appropriate resources.

• Work in partnership with an interdisciplinary care team to facilitate care planning and decision-making.

• Handle your own caseload within a structured virtual environment, achieving set performance targets.

⛳️ Requirements

• Valid, unrestricted RN license in Pennsylvania.

• Associate's or bachelor's degree in nursing.

• Experience in case management, care coordination, or managed care settings is preferred.

• Strong dedication to working with diverse and complex populations.

• Proficient with Microsoft Office applications.

• Bilingual skills are regarded as a significant advantage.
