This is a fully remote position, open to applicants in Texas.

πŸ“‹ Description

β€’ Engage with referred and eligible patients to present care management services, secure verbal enrollment consent, and record details in the EHR.

β€’ Conduct outgoing calls to assist patients in managing chronic diseases through education on conditions and treatments, medication oversight, and appointment scheduling.

β€’ Facilitate coordinated care for patients with chronic cardiovascular issues in collaboration with cardiologists, advanced practice providers, and multidisciplinary teams.

β€’ Evaluate patient health and social determinants of health needs, documenting and implementing care plans accordingly.

β€’ Act as a liaison for interdisciplinary teams and community organizations across both inpatient and outpatient care settings.

β€’ Deliver patient- and family-centered care in outpatient and telehealth environments, including triaging calls and EHR messages.

β€’ Inform patients and caregivers about health conditions, care processes, transitions of care, and the management of chronic care.

β€’ Assess remote patient monitoring technology, diagnostics, and lab results, engaging advanced practice providers for suitable interventions.

β€’ Ensure compliance with CMS documentation and billing standards in the EHR.

β€’ Exceptional clinical, communication, and organizational abilities.

β€’ Capability to multitask and conduct critical analyses.

β€’ Ability to operate independently within the scope of the position.

β€’ Proficiency in functioning within a team environment.

β€’ Knowledge and expertise in care management, encompassing transitional and complex care management.

β€’ Familiarity with basic cardiovascular technology, including EKG, patient assessment, care management, laboratory work, and hospital test outcomes.

β€’ Strong comprehension of population health principles and clinical documentation requirements.

β€’ Interpersonal skills that foster approachability and clarity for patients, families, and care team members.

β€’ Minimum of an associate's degree in nursing from an accredited nursing program.

β€’ Current registered nurse license with a compact license or licensure for Alabama, Arizona, Kansas, Louisiana, Georgia, or Texas.

β€’ Current BLS for Healthcare Providers certification.
