This is a fully remote position, open to applicants in Pennsylvania.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex health issues such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with patient preferences, goals, and requirements.
• Develop nutrition prescriptions that are aligned with clinical diagnoses and foster sustainable behavior change.
• Accurately document all patient interactions in the electronic health record (EHR) in a clear and timely manner.
• Work in collaboration with care teams, including physicians and health coaches, to enhance coordinated patient care.
• Engage in the development of educational content for outreach, engagement, and training initiatives.
• Participate in ongoing quality improvement efforts and innovate care delivery methods.
• Implement current nutrition research and evidence-based practices in patient care.
• Maintain all necessary professional credentials, licenses, and commit to ongoing education.
• Participate in cross-functional meetings and contribute to strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing AND a Master's Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status obtained before the Master's degree requirement takes effect (January 1, 2024).
• Licensed to practice in the state of Pennsylvania.
