Registered Dietitian, Diabetes Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Collaborate closely with Form Health's obesity medicine specialists, including physicians, endocrinologists, PAs, and NPs, to deliver intensive and ongoing medical nutrition therapy support to patients through regular video consultations.

• Build a professional and effective rapport with all patients by employing non-judgmental interpersonal skills to connect with them, ensuring that the care provided is comprehensive, empathetic, and grounded in evidence-based practices.

• Develop tailored nutrition plans for patients, taking into account their unique needs and circumstances.

• Offer nutrition education and counseling focused on diabetes self-management (primarily Type 2 Diabetes), healthy eating practices, and behavior modification strategies.

• Serve as a clinical expert in diabetes management, including interpreting data from Continuous Glucose Monitors (CGMs) or glucometers.

• Assist patients with complex co-morbidities, such as Chronic Kidney Disease (CKD) and cardiovascular conditions.

• Review and provide constructive feedback on patients’ daily food photographs.

• Facilitate patient support between visits through daily text messaging using the Form Health app.

• Contribute to the growth and improvement of our cardiometabolic program in a dynamic and fast-paced environment.

⛳️ Requirements

• Must be a Registered Dietitian with a minimum of 3-5 years of experience in counseling, behavior modification techniques, and working with patients managing diabetes (primarily Type 2 Diabetes).

• Certification as a Diabetes Care and Education Specialist (CDCES) is highly preferred.

• Experience in medical weight loss environments, working with patients on weight loss medications, implementing meal replacement plans, leading support groups, and employing strong cognitive-behavioral therapy counseling techniques.
