This is a fully remote position, open to applicants in United States.
• Deliver skills-focused individual and group sessions to patients and caregivers undergoing treatment at Equip.
• Apply motivational techniques with patients and caregivers to foster engagement with their care plans, including monitoring adherence and completion of program elements.
• Participate in multidisciplinary treatment team huddles to facilitate patient progress in their treatment journey.
• Actively listen and guide patients and caregivers to foresee challenges and implement coping-ahead strategies.
• Maintain an availability calendar to enable patients and caregivers to schedule appointments effectively.
• Conduct proactive outreach (phone calls/messaging), recover missed appointments, provide reminders, track progress, and offer support to ensure full engagement with the care plan.
• Engage in problem-solving to promote adherence to care plans, including identifying and providing practical solutions to obstacles hindering treatment adherence.
• Personalize care plans by making program recommendations and coordinating services to meet patient needs, such as booking additional care team member sessions, registering for groups, providing meal support sessions, and referring personalized resources for adult patients when applicable.
• Establish a relationship grounded in respect and trust with patients.
• Timely document caregiver and patient interactions in compliance with Equip policies.
• Attend and actively participate in internal meetings, as well as initial and ongoing training as assigned.
• Perform other assigned duties.
• Lived experience in eating disorder recovery, with the ability to identify as being in a solid recovery space.
• A minimum of 2 years of experience working directly with patients in a setting that employs protocolized care – including, but not limited to, coaching, case management, mentorship, social work, or milieu therapy.
• Demonstrated comfort working within structured care models or evidence-based protocols (e.g., adhering to specific clinical guidelines, behavior change frameworks, or care plans).
• Proven capability to balance structured protocols with personalization, meeting patients where they are while keeping them aligned with their care goals.
• A strong track record of accountability for patient care plans, utilizing evidence-based frameworks to maintain patient progress and meet clinical milestones.
• Experience in guiding patients and families toward achieving treatment goals while identifying barriers and problem-solving to overcome them.
• Ability to connect with a diverse range of individuals who possess varying lived experiences.
• Passion for evidence-based care and a commitment to learning, practicing, and developing new coaching skills.
• Excellent organizational and time management skills with a proven history of achieving goals.
• Comfortable working with and adapting to new technologies.
• Comfortable with remote work, including the requirement to be on camera.
• Ability to navigate ambiguity and think critically to effectively manage challenges in a dynamic, fast-paced environment.
• Capability to provide 33 hours per week of direct patient care, including sessions for new patients and follow-ups.
• Willingness to commit to working two evenings until 8 PM of your choice each week.
