Quality Assurance Nurse, RN/LPN – Home Health

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Execute quality assurance audits on patient records, care plans, and clinical documentation.

• Examine and confirm a minimum of 6–8 OASIS assessments daily, based on workload, for accuracy, completeness, and compliance (coding is not required).

• Review and assess at least 30 nursing notes daily, contingent on workload.

• Conduct OASIS record evaluations within the timeframes set by the agency.

• Identify documentation gaps, trends, and risk areas, and propose corrective measures.

• Collaborate with nursing staff and interdisciplinary team members to ensure adherence to Medicare regulations, agency policies, and clinical standards.

• Offer education, guidance, and constructive feedback to clinicians regarding documentation quality and regulatory compliance.

• Engage in quality improvement projects, interdisciplinary meetings, and performance enhancement activities.

• Keep accurate, well-organized, and comprehensive records of all quality assurance efforts.

• Exhibit a thorough understanding of current Medicare Home Health regulations and Conditions of Participation (CoPs).

• Assist the agency in preparing for surveys, audits, and accreditation evaluations.

⛳️ Requirements

• Possession of an active and unrestricted Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

• At least one year of experience in home health is required.

• Extensive experience with OASIS review and compliance in home health documentation.

• Comprehensive understanding of Medicare Home Health regulations and Conditions of Participation (CoPs).

• Demonstrated capability to consistently review a minimum of 6–8 OASIS assessments daily or at least 30 nursing notes daily, depending on workload.

• Strong organizational, analytical, and communication abilities.

• Capability to work independently while effectively collaborating with a multidisciplinary team.
