This is a fully remote position, open to applicants in Arizona, +2 more states.
• Accountable for providing personalized and compassionate medical care to patients, primarily those with COPD and other pulmonary disorders.
• Responsible for patient care, ensuring accurate and up-to-date patient records, scheduling appointments, and managing follow-up care as necessary.
• Collaborate with physician specialists, field nurses, community health workers, and other physicians as part of a team.
• Perform assessments on patients in both their homes and through virtual consultations.
• Deliver evidence-based and timely care that aims to minimize preventable hospitalizations, enhance quality of life, and prioritize patient health and satisfaction.
• Prescribe medication, order diagnostic tests, and coordinate with the patient’s Monogram pulmonologist and physician.
• Uphold the quality of care standards established by the practice.
• Active and unrestricted Registered Nurse and Nurse Practitioner license.
• Board certification from ANCC or AANP.
• Current and unrestricted DEA certificate.
• A minimum of 3 years of experience as a Nurse Practitioner in either an inpatient or outpatient pulmonology setting is required.
• Familiarity with computer-based data management programs, information systems, medical records, and point-of-interview technologies.
• Proficient in using medical instruments and equipment necessary for the role.
• Strong understanding of all federal and state regulations, including HIPAA and OSHA.
• Robust background in patient assessment, diagnosis, treatment, and management of pulmonary diseases.
• Exceptional communication skills, ability to work effectively in a team environment, and adaptability.
