Program Manager, Provider Services

This is a fully remote position, open to applicants in United States.

📋 Description

• Cultivate and maintain a comprehensive understanding of the goals and intricacies associated with complex Value-Based Care Programs and/or agreements, such as:

• CMS Kidney Care Contracting

• IWH Payor Agreements

• IWH Practice Participation Agreements

• Communicate this intricate information clearly and concisely to both internal and external stakeholders through meetings and presentations.

• Provide exceptional customer service to all external and internal stakeholders, ensuring consistent feedback and follow-through.

• Investigate and address any issues or concerns in a timely and effective manner.

• Act as the primary contact and subject matter expert for IWH Network practices.

• Support our nephrology partners in achieving success in Value-Based Care by extracting valuable insights from data and identifying areas for improvement.

• Implement company initiatives and guarantee the timely completion of deliverables.

• Ensure that accurate documentation of provider interactions and/or visits is maintained in our CRM.

• Provide timely training and/or onboarding to Network Providers as new information becomes available.

• Confirm that information presented to providers is both accurate and timely, including the data accessible through our dedicated portal.

⛳️ Requirements

• A Bachelor’s degree is required; an Advanced Degree is preferred.

• Demonstrated experience in collaborating with physicians and practices; nephrology-specific experience is advantageous.

• Exceptional attention to detail and outstanding follow-through.

• Should demonstrate an independent and proactive approach, reflecting a commitment to continuous learning.

• Excellent communication and interpersonal skills for effective collaboration with providers, stakeholders, and internal teams.

• Must be willing to travel up to 50% of the time to practices (specific regions will be discussed during the interview).
