Primary Care Coordinator

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Act as the main point of contact for patients, families, facility partners, and healthcare providers within your designated patient panel.

• Assess and prioritize urgent care requests, STAT tasks, and time-sensitive clinical communications in real-time.

• Ensure precise and compliant patient documentation is maintained within our EMR system.

• Proactively oversee appointment scheduling, follow-up care, and interdisciplinary communication.

• Facilitate new patient onboarding by ensuring each patient is contacted and scheduled within 24 hours post-enrollment.

• Participate in after-hours on-call rotation to ensure continuity of care.

• Contribute to initiatives focused on quality improvement and operational efficiency.

⛳️ Requirements

• Minimum of 2 years of direct patient care experience in a clinical environment (e.g., Medical Assistant, LVN/LPN, CNA, Phlebotomist, Patient Care Technician, or similar roles).

• Experience in healthcare coordination, patient services, or clinical operations is required.

• Experience with geriatric, Medicare Advantage, or value-based care is strongly preferred.

• Strong triage and prioritization abilities in high-volume, real-time settings.

• Proficient in EMR/EHR systems and healthcare technology platforms.

• Excellent communication skills with a variety of stakeholders (patients, families, physicians, facility leadership).

• Self-motivated and accountable; capable of working independently in a remote environment.

• Availability for on-call shifts, approximately 5 overnight and/or weekend shifts per rotation.

• Remote work setup requires reliable high-speed internet and a dedicated, HIPAA-compliant workspace.

• Must maintain good standing with OIG, CMS, and Federal Health Care Programs (no exclusions or debarments) for regulatory compliance.
