Population Health Specialist

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Engage in proactive outreach to patients and individuals who could benefit from the services offered by the Health Center.

• Assist patients in understanding and accessing the healthcare services and resources available to them.

• Identify and address gaps in care through outreach, education, scheduling, and follow-up.

• Follow up with patients who miss preventive or other significant appointments and assist them with rescheduling.

• Utilize patient and program data to monitor outcomes, identify trends, and support quality improvement initiatives.

• Support clinical quality measures and value-based care efforts through patient engagement and follow-up.

• Develop trusting relationships with patients and caregivers to encourage ongoing engagement with care.

• Collaborate with healthcare teams to facilitate coordinated, patient-centered care.

• Contribute to program initiatives, performance monitoring, and reporting efforts.

• Maintain precise documentation and adhere to all relevant federal and state privacy regulations.

• Implement approved crisis intervention techniques as necessary, in line with current certification and organizational protocols.

• Perform additional duties as assigned by the Director of Population Health or senior management.

⛳️ Requirements

• High School diploma or GED is required.

• Bachelor’s degree is preferred.

• A minimum of three (3) years of relevant experience is required.

• Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.

• Must obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hiring.

• Ability to conduct outreach, follow-up, documentation, and manage multiple priorities effectively.

• Relationship-centered communication characterized by patience, empathy, and respect.

• Capability to work with data and identify opportunities for enhancing patient engagement and outcomes.

• Organized, reliable, and proactive in follow-through.

• Ability to work collaboratively as part of a multidisciplinary healthcare team.
