This is a fully remote position, open to applicants in North Carolina.
• Exhibit clinical expertise for specific complex and/or rising risk member populations.
• Utilize data informatics to identify members suitable for population health programs.
• Deliver interventions and educational resources to foster health prevention and promotion.
• Enroll selected Trillium members into relevant population health programs.
• Assist in the development and management of future population health programs.
• Engage members by clarifying program mission, goals, and objectives while providing a comprehensive clinical background.
• Collaborate with a multidisciplinary team to create tools and interventions aimed at achieving optimal patient outcomes.
• Coordinate and facilitate the timely execution of assessments, care plans, and necessary interventions in partnership with Care Management staff.
• Adhere to established policies and procedures to attain optimal clinical outcomes and enhance the health status of Trillium members.
• At least one (1) year of experience as a Registered Nurse (RN) working with the BH/MH/SU/IDD population.
• Must hold a valid Registered Nurse (RN) license.
• Required to be fully licensed by the North Carolina State Board of Nursing.
• A valid driver's license is mandatory.
• Candidate must reside within North Carolina.
• Ability to travel within the designated catchment area as needed.
• Preferred: Familiarity with Quality Management, Utilization Management, and discharge planning procedures.
• Preferred: Experience with data analytics for population health management.
• Preferred: Certification as a Managed Care Nurse or in care management.
