Population Health Nurse

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver clinical care management and support for population health for designated members

• Emphasize prevention, wellness, and long-term health achievements

• Utilize nursing judgment to evaluate data, determine care requirements, and coordinate services through population health management strategies

• Perform clinical assessments to discern member health necessities, risks, and care deficiencies

• Create and execute care plans aimed at preventive care, chronic condition management, and wellness objectives

• Facilitate coordination of services among providers, care teams, and community resources

• Track member advancement and modify care plans based on clinical metrics and outcomes

• Offer health education and coaching to foster self-management and involvement

⛳️ Requirements

• Associate’s degree in Nursing

• One (1) year of experience working as a Registered Nurse

• Current North Carolina Licensure as a Registered Nurse (RN)

• Valid driver's license

• Must reside in North Carolina

• Ability to travel within the designated area as necessary

• Experience with BH/MH/SU/IDD populations is preferred

• Familiarity with QM and UM procedures is preferred

• Experience leveraging data analytics for population health management is preferred

• CMCN, CCM, ACM, or similar care-management or nursing certification is preferred

• Must successfully pass a drug test as a condition of employment
