This is a fully remote position, open to applicants in California, +4 more states.
β’ Deliver direct patient care within a telemedicine clinical environment, concentrating on evidence-based strategies for obesity and extensive diabetes management, including insulin titration and care for patients utilizing insulin pumps and/or continuous glucose monitors (CGMs).
β’ Collaborate with Advanced Practice Provider (APP) colleagues and Registered Dietitians who specialize in diabetes treatment, particularly Type 2 Diabetes, to jointly manage a shared panel of patients.
β’ Review patient charts and perform initial and follow-up visits for APP patients every third visit to guarantee quality care, continuity, and effective management of Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Coronary Artery Disease (CAD)/Cardiovascular (CV) conditions.
β’ Provide advanced treatment planning and medical management assistance for intricate endocrinology cases, elevated concerns, and patients necessitating specialized management protocols.
β’ Prescribe non-controlled medications related to obesity and diabetes for patients under direct care as well as in collaboration with APPs.
β’ Hold weekly 30-minute collaborative meetings and 30 minutes of asynchronous communication with each assigned Endocrine-Specialized APP to review cases, tackle clinical challenges, and offer continuous mentorship.
β’ Ensure that clinical documentation, protocols, and treatment pathways are adhered to consistently within the team, focusing on guidelines for managing complex diabetic populations.
β’ Support the professional development of colleagues by encouraging clinical growth, providing constructive feedback, and promoting continuing education in endocrinology and obesity medicine.
β’ Engage in team-based care initiatives and help cultivate a culture of collaboration and high-quality care.
β’ Participate in relevant departmental meetings and contribute to quality improvement initiatives within the diabetes and obesity medicine program.
β’ Board certification in Endocrinology from the American Board of Internal Medicine (ABIM).
β’ An active and unrestricted medical license in at least one of the following states: CA, TX, IL, MA, NY, or in any IMLC-participating state with a valid Letter of Qualification (LOQ) and the capability to obtain additional state licenses.
β’ Current or prior credentialing with insurance payers.
β’ Certification from the American Board of Obesity Medicine (ABOM) or significant clinical experience (minimum of 3 years) in obesity medicine and pharmacotherapy, including both older oral medications and newer injectable options.
β’ At least 5 years of experience as an attending physician in endocrinology.
β’ Proven competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
β’ Expertise in advanced diabetes management, including the usage of CGMs, insulin pumps, and intricate insulin regimens.
β’ A strong desire to mentor, teach, and work collaboratively with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes.
β’ Demonstrated experience working with APPs, medical residents, or trainees within a clinical academic environment.
β’ Exceptional communication and interpersonal abilities.
β’ Commitment to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity.
