This is a fully remote position, open to applicants in Virginia.
• Conduct initial and ongoing comprehensive assessments of client needs, including Outcome and Assessment Information Set (OASIS) assessments.
• Collaborate with the physician to establish and revise a treatment plan as needed.
• Support the development and implementation of the interdisciplinary client care plan related to Physical Therapy.
• Assist the physician in evaluating clients through diagnostic and prognostic tests assessing muscle, nerve, joint, and functional abilities.
• Provide treatment to alleviate pain, restore functionality, and maintain optimal performance through physical interventions, such as exercise.
• Educate and engage staff regarding the developed treatment plan.
• Teach, supervise, and guide the client and their family in the physical therapy program.
• Instruct clients on the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Supply the necessary equipment to execute the treatment plan.
• Coordinate out-client services for procedures that cannot be conducted at home.
• Timely document all evaluation data, treatments, and client responses to therapy interventions, including quality assurance data.
• Prepare assessments of client conditions along with periodic evaluations and summaries.
• Supervise Physical Therapy Assistants and paraprofessionals in accordance with agency policy.
• Document and report client reactions to treatment and any changes in their condition to the physician.
• Maintain communication with the physician at least every 30 days or whenever there are changes in the client’s status, obtaining additional orders as needed.
• Collaborate with other team members (including home attendants) and, when appropriate, provide instruction on certain aspects of physical therapy relevant to client care.
• Write clinical and progress notes on the day of the visit, incorporating them into the client's record within 72 hours.
• Engage in case conferences, physical therapy staff meetings, agency activities, and in-service programs as required.
• Must be a graduate of a Physical Therapy program recognized by the Council of Medical Education of the A.M.A. in partnership with the American Physical Therapy Association.
• Must hold a license or registration in Virginia.
• At least one year of institutional or home health experience is required.
• Current CPR certification is required.
• Familiarity with Medicare regulations and guidelines is advantageous.
• A minimum of one year of experience as a Physical Therapist is required, with a strong preference for home health experience.
• Proficiency in using Physical Therapy equipment is essential.
• Ability to provide two references is necessary.
• Must successfully pass OIG checks, Virginia State Police background checks, and national background checks, and verify employment eligibility with the required documentation.
• Reliable transportation, proof of valid car insurance, and a valid driver’s license are required.
