Pharmacist – Medication Therapy Management

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Conduct thorough telephonic medication reviews and reconciliations for targeted high-risk members.

• Authenticate and engage members via HIPAA for comprehensive medication reviews.

• Update member contact information and record medication allergies and side effects.

• Verify prescriptions, vitamins, supplements, and over-the-counter medications.

• Identify, document, and facilitate the resolution of medication-related issues.

• Produce written patient takeaway summaries that are tailored to an appropriate health literacy level.

• Prepare faxed materials for prescriber offices to assist in resolving medication-related issues.

• Establish and nurture positive relationships with physicians, nurses, internal resources, clients, patients, prescribers, and nurse case managers.

• Formulate member-specific treatment plans in accordance with nationally recognized treatment guidelines.

• Complete internal training on behavioral modification and interviewing, applying these skills to enhance patient engagement and the completion of comprehensive medication reviews.

• Undertake additional responsibilities as assigned by the supervisor and/or manager.

⛳️ Requirements

• Prior pharmacy experience providing MTM, specifically utilizing the Outcomes MTM Connect platform.

• At least 7 months in the MTM Ops department with a record of meeting expectations in all core metrics.

• Experience in a call center environment.

• Ability to maintain focus and achieve goals while working from home.

• Excellent verbal and written communication skills.

• Basic computer proficiency and familiarity with Microsoft Office products.

• Outstanding prioritization and organizational abilities.

• Demonstrates integrity and sound judgment in handling confidential information.

• Applicants must reside in and hold an active RPh license in the state of PA, IL or FL.

• Bachelor’s degree in Pharmacy or PharmD.

• Active pharmacist license in good standing.
