Pharmacist – Medication Therapy Management

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Provide telephonic medication therapy management (MTM) for high-risk members.

• Conduct telephonic comprehensive medication reviews and medication reconciliations.

• Perform clinical assessments for relevant conditions.

• Engage and authenticate members via HIPAA for comprehensive medication reviews.

• Update member addresses and record medication allergies and side effects.

• Verify prescriptions, vitamins, supplements, and over-the-counter medications.

• Identify, document, and assist in resolving medication-related issues.

• Develop written patient takeaway summaries tailored to an appropriate health-literacy level.

• Prepare faxed materials for prescriber offices.

• Establish and maintain relationships with physicians, nurses, members, caregivers, prescribers, clients, and internal resources.

• Formulate member-specific treatment plans in line with nationally recognized treatment guidelines.

• Complete internal training and utilize behavioral modification and interviewing techniques.

• Carry out additional responsibilities as assigned by the supervisor or manager.

⛳️ Requirements

• Prior pharmacy experience in delivering MTM, particularly using the Outcomes MTM Connect platform.

• Minimum of 7 months experience within the MTM Ops department and meets expectations in all core metrics.

• Previous experience in a call center environment.

• Ability to maintain focus and achieve goals while working from home.

• Strong spoken and written communication skills.

• Basic computer proficiency and familiarity with Microsoft Office products.

• Outstanding prioritization and organizational capabilities.

• Demonstrates integrity and sound judgment when handling confidential information.

• Candidates must reside in and hold an active RPh license in PA, IL, or FL.

• Bachelor of Science in Pharmacy or PharmD.

• Active pharmacist license in good standing.
