Pharmacist – Medication Therapy Management

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Provide telephonic medication therapy management (MTM) services to high-risk members.

• Conduct telephonic comprehensive medication reviews and carry out medication reconciliations.

• Perform clinical assessments for relevant conditions.

• Engage with members and authenticate their identity in compliance with HIPAA for thorough medication reviews.

• Update member mailing addresses and document any medication allergies and side effects.

• Verify prescriptions, vitamins, supplements, and over-the-counter medications.

• Identify, record, and facilitate the resolution of medication-related issues.

• Develop written takeaways for patients that are tailored to their health literacy levels.

• Create faxed documents for prescriber offices to aid in resolving medication-related issues.

• Foster and maintain positive relationships with physicians, nurses, and both internal and external clients.

• Collaborate with patients, prescribers, nurse case managers, clients, and internal resources to formulate member-specific treatment plans.

• Utilize nationally recognized treatment guidelines.

• Complete internal training focused on behavioral modification and interviewing techniques.

• Undertake other tasks as assigned by the supervisor and/or manager.

⛳️ Requirements

• Prior pharmacy experience providing MTM, particularly with the Outcomes MTM Connect platform.

• Minimum of 7 months in the MTM Ops department, consistently meeting expectations in all core metrics.

• Previous experience in a call center setting.

• Ability to maintain focus and achieve goals while working from home.

• Excellent verbal and written communication skills.

• Basic computer skills and proficiency in Microsoft Office applications.

• Outstanding prioritization and organizational capabilities.

• Demonstrates integrity and sound judgment when handling confidential information.

• Applicant must reside in and hold an active RPh license in the state of PA, IL, or FL.

• Bachelor’s degree in Pharmacy or PharmD.

• Active pharmacist license in good standing.
