This is a fully remote position, open to applicants in Arizona.
• This role allows for 100% remote work after completing a 90-day training period and meeting performance expectations.
• Medication Therapy Management (MTM) pharmacists with advanced qualifications, Board Certifications, or similar credentials are sought to operate in a progressive environment where they provide MTM services directly to patients.
• The clinical pharmacist collaborates within a multidisciplinary team and works alongside case managers.
• Consultations are conducted one-on-one via phone or video with members of commercial and Medicare Part D plans.
• The pharmacist may assist in the intake/enrollment process.
• Responsibilities include consulting with patients to identify, resolve, and prevent medication-related issues.
• Each patient's medications are assessed for indications, effectiveness, safety, and adherence, incorporating lab data into the evaluation when available.
• A medication summary and personal care plan with recommendations will be prepared for patients to share with their healthcare providers.
• The pharmacist will follow up with patients to monitor their drug therapy and ensure positive outcomes as necessary.
• Must hold an Arizona Pharmacist License in good standing.
• Residency in the State of Arizona is required.
• Exceptional verbal and written communication skills are essential.
• Must be well-organized with the ability to prioritize tasks effectively.
• A strong customer-focused mindset is necessary.
• Candidates should be ownership and results-driven.
• Attention to detail is a must.
• Proficiency in Microsoft Office applications, such as Outlook, Excel, and Word is required.
• Capability to work effectively within a team is essential.
• Flexibility in adapting to change is important.
• Must be able to effectively plan, prioritize, and organize time and workload.
• Skills in identifying and resolving issues, along with recommending process improvements, are important.
• Ability to thrive in a deadline-oriented environment is necessary.
• A Pharm D Degree is required.
