This is a fully remote position, open to applicants in United States.
• Deliver patient education through telephonic consultations for specialty pharmacy clients.
• Evaluate and communicate disease state management, strategies for side effect mitigation, medication administration, storage, and therapeutic suitability.
• Respond to clinical inquiries from patients, caregivers, and the pharmacy team.
• Record patient consultations and interventions in the pharmacy software system.
• Interpret and conduct clinical evaluations of specialty medication prescriptions.
• Reach out to providers and patients to confirm prescription details and expedite order processing.
• Validate and confirm the legitimacy of controlled substances.
• Liaise with physicians for new prescriptions and consult on pharmaceutical inquiries with physicians and nurses.
• Coordinate with pharmacies regarding prescription transfers.
• Convert prescriptions to generic, biosimilar, or preferred medications whenever feasible.
• Ensure accuracy of prescription information entered by order entry.
• Adapt to new workflows and address unique pharmaceutical challenges.
• Stay updated on specialty pharmacy therapies, drug approvals, and treatment guidelines for various disease states.
• Provide backup support and training for fellow pharmacists as required.
• Document and report any medication errors.
• Promote error reduction and prevention strategies in line with quality department and ISMP standards.
• Adhere to relevant federal and state laws and uphold ethical standards.
• Achieve department production metrics, service levels, and quality benchmarks.
• Address patient concerns and adjust priorities according to business needs.
• Graduate from an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Hold a current, unrestricted pharmacist license in the state of residence; must continually pursue educational credits to maintain active licensure.
• Strong clinical background is essential.
• A minimum of two years of experience as a practicing licensed Pharmacist is preferred.
• Experience and/or a thorough understanding of Specialty Pharmacy medications is desirable.
• Participate in, adhere to, and support compliance program goals.
