This is a fully remote position, open to applicants in Florida.
• Manage after-hours phone calls and effectively address callers' needs.
• Offer telephone guidance and education tailored to patient requirements.
• Respond to telephone inquiries regarding the services of Chapters Health System.
• Advocate for and elucidate the advantages of Chapters Health System’s services.
• Collect essential information and documentation to ensure complete and accurate patient records.
• Input and monitor patient referral information within the EMR.
• Provide comfort and reassurance to patients and their families over the phone.
• Assist in resolving inquiries and recognize when an in-person visit is necessary.
• Organize in-person visits as needed or as requested.
• Support patients and families in attaining their desired comfort levels.
• Utilize support and expert resources to address complex situations.
• Log patient and family interactions in the EMR and coordinate with the Interdisciplinary Group.
• Immediately report technical issues and strive for resolution within 30 minutes.
• Escalate to an IPU or Resource Center if technical issues remain unresolved, as instructed.
• Report to a designated company location during emergencies to ensure service continuity.
• Undertake additional responsibilities as assigned.
• Active Florida or multi-state RN license.
• Ability to apply for or secure a multi-state RN license within 30 days of hiring if not already in possession.
• Florida RN license, an active single-state out-of-state nursing license, or a current active multistate/compact nursing license.
• Capability to apply for or obtain an RN license in Washington, D.C. within 30 days of hiring.
• At least one year of nursing experience required.
• Preferred experience in hospice or hospital settings.
• Familiarity with an EMR/EHR system is advantageous.
• Effective verbal and written communication skills in English.
• Bilingual in Spanish is a plus.
• Reliable transportation to accommodate patient needs.
• Excellent critical thinking, problem-solving, clinical, and assessment abilities.
• Capacity to operate equipment with visual and auditory components.
• Strong time management and prioritization skills.
• Ability to handle stressful situations with professionalism and composure.
• Proficient in patient service, information-seeking, listening, and knowledge transfer.
• Capability to thrive in a fast-paced environment.
• Willingness to collaborate effectively in a team setting.
• Proficient in computer skills, including MS Office, online research, data entry, and navigation.
• Availability to work scheduled shifts during extended operational hours.
• Ability to perform sedentary tasks with frequent interruptions.
• Capacity to manage emotional stress when caring for terminally ill patients and their families.
• Ability to carry out essential physical tasks, such as lifting patients or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodations.
• Successful completion of position competency requirements.
• Agreement to drug and/or alcohol testing following a conditional offer and ongoing adherence to the Drug-Free Workplace Policy.
• Verification of eligibility through the Florida Care Provider Background Screening Clearinghouse.
• $45.00 hourly pay rate.
