Per Diem Licensed Practical Nurse, Brain Injury

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Engage in thorough physical health evaluations at the time of admission and whenever there are changes in medical conditions.

• Deliver nursing care, education, and support with daily living activities in accordance with established care plans.

• Administer wound care, diabetes management, oral care, foot care, GT/JT care, nutritional oversight, and catheter management.

• Execute Individualized Action Plans (IAPs) utilizing recovery-focused, motivational interviewing, harm-reduction, trauma-informed, culturally aware, and strengths-oriented rehabilitation methodologies.

• Observe clients and relay any changes in their status to program managers, primary care providers, family members/guardians, and external organizations.

• Conduct personalized assessments and provide expert nursing interventions.

• Measure and document vital signs.

• Gather sputum, urine, and stool samples as per medical orders.

• Dispense medications and administer treatments as prescribed.

• Ensure the availability and upkeep of medical equipment and supplies.

• Transport and accompany clients to medical, dental, and other necessary appointments.

• Engage in training sessions, supervision meetings, and staff gatherings.

• Adhere to company policies, safe work practices, licensing standards, accreditation guidelines, and relevant regulations.

• Contribute to quality improvement objectives.

• Utilize crisis intervention techniques, access on-call support, and complete documentation for incidents.

• Ensure the maintenance and safety of the physical facility.

• Perform additional related duties as necessary.

⛳️ Requirements

• Associate's degree in Nursing is required.

• A high school diploma or GED is mandatory.

• Must possess a current L.P.N. license valid in Massachusetts.

• Experience with individuals who have suffered brain injuries is highly preferred.

• Strong skills in time management and written communication are essential.

• Applicants must be at least 21 years of age.

• A current, valid US driver's license held for a minimum of 6 months is required; later requirements will necessitate at least one year.

• Must obtain CPR certification within two weeks of employment.

• Must achieve Safety Care certification within 90 days of employment.

• Successful completion of CORI, reference, and national fingerprinting background checks is required.

• Familiarity with empowerment and self-advocacy strategies is essential.

• Sensitivity to cultural, religious, ethnic, disability, and gender-related issues is important.

• Knowledge of human, legal, civil rights, community resources, and other relevant information is necessary.

• Awareness of various communication and learning styles is required.

• Proficiency in crisis intervention techniques is important.

• Ability to manage competing priorities effectively.

• Capacity for making independent judgments and decisions.

• Ability to maintain a professional and confidential demeanor.

• Familiarity with personal computer applications and equipment is required.

• Capability to meet physical demands such as standing, walking, bending, kneeling, stooping, crouching, crawling, climbing, lifting at least 25 pounds, assisting with daily living activities, and communicating with clients and staff.

• A valid LPN license in the state of service is required.
