This is a fully remote position, open to applicants in Michigan.
• Support care coordination initiatives and address needs for a designated group of paneled patients.
• Work collaboratively with cross-functional teams to resolve issues and meet objectives.
• Serve as a liaison between providers and patients by communicating messages, processing referrals and faxes, and managing prior authorizations.
• Assess the effectiveness of interventions and treatments, recommending adjustments to care plans as necessary.
• Collaborate with the integrated care team to address medical concerns effectively.
• Advocate for patients during High-Risk Huddles, providing assessments and actionable recommendations.
• Reach out to patients discharged from hospitals or emergency departments to gather information and close care gaps.
• Identify, document, and address issues, goals, and interventions in care plans.
• Relay clinical information and instructions to patients, caregivers, facilities, and other entities on behalf of providers.
• Engage in collaborative care teams and High-Risk Huddles to enhance quality and patient experience outcomes.
• Identify and work towards the resolution of Social Determinants of Health barriers.
• Proactively oversee patient care through outreach and scheduled follow-up checkpoints.
• Promptly document calls and patient interactions in the EHR in accordance with clinical documentation guidelines and relevant laws.
• Interact compassionately and professionally with patients, families, care teams, and external entities.
• Recognize, escalate, and address patient complaints, grievances, and concerns effectively.
• Comply with HIPAA regulations and all applicable state and federal laws.
• High school diploma or GED required.
• A minimum of 2 years of experience in care coordination or healthcare.
• Familiarity with Medicare, Medicare Advantage, and Medicaid insurance programs.
• Well-organized, detail-oriented, and capable of multitasking in a fast-paced, dynamic environment.
• Strong interpersonal communication and active listening skills.
• Adaptability to meet changing community needs and program environments.
• Excellent critical thinking and sound judgment abilities.
• Proven time-management skills.
• Strong dedication to patient advocacy.
• Self-motivated with outstanding problem-solving capabilities.
• Basic knowledge of medical terminology.
• Ability to work both independently and collaboratively within a team.
• Proficiency in EMR software/applications, call center technology, and Microsoft Office Suite.
• Experience in community work or healthcare settings.
• Understanding of fundamental principles, practices, and techniques in primary healthcare.
• Active LPN/LVN license in the state where patients are located, or certification as a Foreign Nursing Graduate, Foreign Medical Graduate, or Community Health Worker (preferred).
• Experience with Athena EHR (preferred).
• At least 2 years of experience providing exceptional customer service (preferred).
• A minimum of 2 years of experience in a role requiring critical thinking (preferred).
