Patient Flow Nurse – Fixed-term

This is a fully remote position, open to applicants in Canada.

📋 Description

• Manage and oversee the scheduling of virtual appointments based on provider availability and patient preferences.

• Coordinate patient flow and assess healthcare needs to prioritize appointments and visits.

• Evaluate patient needs across different sites and ascertain the appropriate level of care required.

• Work collaboratively with healthcare providers to enhance virtual care schedules and prioritize appointments effectively.

• Relay patient-specific information and instructions to providers prior to scheduled virtual appointments.

• Ensure that medical records, appointment details, and outcomes of virtual visits are kept accurate and up-to-date.

• Create and distribute virtual care reports to relevant stakeholders.

• Guarantee that patients’ lab results are communicated in accordance with established procedures.

• Adhere to processes for conveying critical lab results.

• Collaborate with healthcare team members to maintain continuity of care during transitions from virtual to in-person services.

⛳️ Requirements

• Bachelor's degree in nursing or equivalent professional experience.

• Must be in good standing with the governing College.

• Proficient in CTAS (Canadian Triage and Acuity Scale) within a high-paced environment.

• Compliance with the Emergency Nursing Scope and Standards of Canadian Practice (National Emergency Nurses Association, 2018).

• Proven experience in healthcare coordination, scheduling, and administration.

• Current CTAS certification with over 5 years of experience.

• Familiarity with electronic health record (EHR) systems.

• Prior experience with telemedicine or virtual care services.

• Strong organizational and time management abilities.

• Excellent communication skills for effective interaction with patients, healthcare providers, and administrative staff.

• Understanding of virtual care platforms and basic technical troubleshooting.

• Knowledge of medical terminology and fundamental healthcare processes.

• Capability to work independently as well as part of a healthcare team.

• High attention to detail and accuracy in managing patient records and documentation.

• Empathy and compassion when addressing patients’ health concerns and needs.

• Awareness of healthcare compliance and regulations regarding patient privacy.

• Multiple provincial nursing licenses are advantageous.

• Strong written, verbal, and documentation skills.

• Ability to handle multiple tasks efficiently.

• Strong interpersonal skills.

• Full availability from Monday to Sunday, including day, evening, night, and weekend shifts.
