Patient Care Coordinator

This is a fully remote position, open to applicants in United Kingdom.

📋 Description

• Reach out to patients post-referral to advocate for the Diabetes Remission Programme.

• Offer guidance, support, resources, and deliver exceptional customer service to patients.

• Conduct initial lifestyle assessments during booking conversations.

• Assess patients for additional coaching support when necessary.

• Independently oversee and manage waiting lists based on session availability.

• Address incoming inquiries and enroll service users into the programme.

• Assist patients throughout their journey within the programme.

• Organize and coordinate bookings for both individual and group sessions.

• Ensure a seamless and positive experience for patients.

• Prior experience in customer care, preferably in a call centre, complaints handling, or hospitality setting.

• Capability to handle challenging customer interactions and deliver effective solutions.

• Strong written and verbal communication abilities.

• Positive, solution-focused mindset and work approach.
