Part-Time Nurse Practitioner

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Provide preventive healthcare services to Clover members remotely through Clover’s proprietary software platform and other approved video applications.

• Conduct a thorough review and evaluation of a member's medical history, physical health, environment, and psycho-social status.

• Record progress notes, prepare member workups, and create encounter summaries utilizing Clover Assistant and EMR.

• Assist at-risk members in transitioning from hospitals and other facilities to their homes.

• Develop personalized care plans in collaboration with the member's primary care physician.

• Actively address gaps in care, which may involve ordering laboratory tests, writing prescriptions in conjunction with the Primary Care Provider or collaborating physician, and facilitating follow-up with physicians.

• Conduct point-of-care laboratory tests and diagnostic evaluations, such as A1c, spirometry, and other relevant tests.

• Engage in activities that support the goals of the Clover Care Visit program, including accurate documentation of member health conditions, achieving high performance on federal quality measures, and successful referrals to network providers and internal Clover clinical teams to enhance care and outcomes.

• Ensure that completed visits include member-specific AthenaNet and Clover Assistant (CA) documentation that is finalized, electronically signed, and closed in accordance with Clover policy:

• All appropriate AthenaNet visit documentation tabs must be filled out based on visit type.

• All relevant orders, referrals, care gaps, PCP letters, follow-up visit sections, and tasks related to clinical inbox and care coordination must be completed.

• Visit-specific CPT codes should be included in the AthenaNet Billing Tab.

• All CA visit tasks must be addressed, signed, and automatically loaded into the AthenaNet visit.

• Collaborate and communicate effectively with members’ interdisciplinary care teams.

• Participate in continuous training and remote meetings.

• Adhere to all Clover Care Service policies, procedures, standards of care, and workflows.

• Perform additional duties as assigned.

⛳️ Requirements

• Must be based in the state of New Jersey.

• Certification as an Advanced Nurse Practitioner in specialty areas is required.

• Possession of a current, unencumbered NJ Licensure as a Registered Nurse or a valid medical doctor license in New Jersey is mandatory.

• Active DEA and CDS registrations are required.

• A minimum of 1 year of clinical experience providing direct patient care is necessary.

• Graduation from an accredited nursing school with a Bachelor’s degree (BSN) is required.

• A current provider card in Basic Life Support (BLS) is necessary.
