This is a fully remote position, open to applicants in Ohio.
• Perform thorough in-home health risk evaluations to identify both active and chronic health conditions, as well as assess all physical, mental, and social needs at the time of the assessment.
• Gather patient history, conduct examinations, and establish diagnoses.
• Provide written documentation of patient visits in accordance with NCQA standards.
• Measure patient vital signs as required.
• Initiate case management referrals and communicate with primary care providers as needed.
• Collaborate with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnoses.
• Execute all clinical responsibilities while adhering to OSHA Universal Precautions.
• Uphold patient confidentiality at all times.
• Attend mandatory meetings and in-service programs and participate in committees as requested.
• Engage in professional development activities and maintain required professional licenses and affiliations.
• Active/unrestricted nurse practitioner license valid in the states of coverage.
• Board certification from one of the following organizations: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).
• Current Basic Life Support (BLS) certification.
• Active enrollment in Medicare/Medicaid programs.
• Must possess a valid driver's license and maintain a good driving record.
• Exceptional skills in Electronic Health Records (EHR).
