Nurse Practitioner / Physician Assistant / Certified Nurse Midwife

This is a fully remote position, open to applicants in Texas.

📋 Description

• Track trends in patient data (such as blood pressure, weight, and symptoms) to identify high-risk cases that need intervention.

• Conduct clinical assessments through phone, telehealth, or messaging to offer education, interventions, and adjustments to care plans.

• Deliver direct patient education regarding the management of hypertension, gestational diabetes, risks associated with preeclampsia, postpartum recovery, and other maternal health issues.

• Refer high-risk cases to the appropriate provider following established protocols.

• Assist in care coordination by helping patients with referrals, scheduling follow-ups, and connecting them to additional resources.

• Ensure adherence to RPM protocols by coaching patients and addressing any barriers to engagement.

• Record all clinical interactions in Babyscripts' care management platform, ensuring accuracy and compliance with healthcare regulations.

⛳️ Requirements

• A valid, unrestricted NP, PA, or CNM license in the state of Texas (mandatory).

• At least 3–5 years of clinical experience in OB/GYN, women’s health, maternal-fetal medicine, or related fields.

• Previous experience with remote patient monitoring, telehealth, or population health is preferred.

• Strong ability to analyze data and communicate clear clinical insights.

• Proficiency in navigating EHRs, care management platforms, and digital tools.

• Outstanding written and verbal communication skills.

• Bilingual (Spanish) is highly preferred.
