
Nurse Practitioner – National After-Hours Team – PRN

This is a fully remote position, open to applicants in North Carolina, +1 more state.
• Act as the primary support resource for residents and facility personnel during after-hours, offering guidance and medical assistance via phone.
• Utilize Curana’s telephonic system to manage calls, facilitating care coordination among facilities, hospitals, and clinics.
• Provide high-quality, cost-efficient care addressing acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.
• Conduct thorough assessments and accurately document interactions in the EMR, ensuring adherence to CMS standards.
• Implement Curana’s clinical protocols and practice guidelines to ensure safe and effective treatment is provided whenever feasible.
• Conduct a minimum of 30% of consultations via telehealth, including evaluations for acute changes, falls, and controlled substance assessments.
• Independently handle multiple calls across various states.
• Engage in required education and training to remain informed on care standards.
• Provide coverage during weeknights, weekends, and holidays as per the established schedule.
• Deliver 48 hours of coverage monthly, which includes 36 hours during weekends.
• Master’s Degree in Nursing as a Nurse Practitioner.
• Active licensure in North Carolina and South Carolina is mandatory.
• Additional active licenses in Florida and Georgia are highly preferred.
• Willingness to acquire further state licenses within a 30-day period as required.
• National Nurse Practitioner certification as ANP, FNP, or GNP.
• Capability to secure DEA licensure / Prescriptive Authority.
• Experience in acute and chronic disease management.
• Clinical expertise in adult, family, or geriatric care.
• Over 3 years of experience as a Nurse Practitioner.
• Ability to establish a collaborative practice agreement, if required in the relevant state(s).
• Capacity to work scheduled shifts in line with scheduling policies.
• Proficient in computer skills, including recording medical data in both written and electronic formats.
• Preferred experience in nursing homes or with seniors in acute care settings.
• Familiarity with geriatrics, chronic illnesses, and acute disease management is preferred.
• Knowledge of advanced illness and end-of-life conversations is preferred.
• Ability to cultivate and sustain positive relationships with customers.
• Flexibility in adapting to change.
• Must be available for evening, night, weekend, and holiday shifts.
