Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Act as the primary support for residents and facility personnel during after-hours, offering guidance and medical assistance via phone.

• Utilize Curana’s telephonic system to manage calls, ensuring coordination of care among facilities, hospitals, and clinics.

• Provide high-quality, cost-efficient care that addresses acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct thorough assessments and accurately document encounters in the EMR, maintaining compliance with CMS regulations.

• Implement Curana’s clinical protocols and practice guidelines to ensure safe and effective treatment in situ whenever feasible.

• Engage in mandatory education and training to remain updated on care standards.

• Offer coverage during weeknights, weekends, and holidays.

• Handle high call volumes and multiple calls autonomously.

• Conduct at least 30% of visits through telehealth, including assessments of acute changes, falls, and controlled substance consultations.


⛳️ Requirements

• Master’s Degree in Nursing as a Nurse Practitioner.

• Active licensure in North Carolina and South Carolina is required.

• Additional active licenses in Florida and Georgia are highly preferred.

• Willingness to secure additional state licenses within a 30-day period as necessary.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP).

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical expertise in adult, family, or geriatric care.

• Minimum of 3 years of experience as a Nurse Practitioner.

• Ability to establish a collaborative practice agreement if required in your state(s).

• Availability to work scheduled shifts according to scheduling policies.

• Proficient in computer skills, including the documentation of medical information in both written and electronic medical records.

• Previous experience in a nursing home or with elderly patients in an acute care setting is preferred.

• Familiarity with geriatrics, chronic conditions, and acute disease management is preferred.

• Understanding of advanced illness and end-of-life conversations is preferred.

• Ability to cultivate and maintain positive customer relationships.

• Flexibility to adapt to changes.

• Must be able to work 48 hours monthly, including 36 hours on weekends.

• The company is not able to offer visa sponsorship at this time.
