This is a fully remote position, open to applicants in Michigan.

📋 Description

• Facilitate virtual onboarding and follow-up visits with the assistance of a Health Advocate to conduct thorough biopsychosocial assessments, diagnose and manage chronic conditions in accordance with evidence-based guidelines, perform preventive screenings, and address preventive and quality care gaps.

• Address both medical and behavioral health diagnoses and treatment plans, including chronic disease management, acute medical issues, depression, anxiety, bipolar disorder, PTSD, schizophrenia, substance use disorders, and various other mental health disorders.

• Ensure that every patient receives face-to-face consultations on an annual basis and proactively schedules additional visits as clinical needs or care gaps arise.

• Order and monitor lab tests and imaging studies to enhance diagnosis, prevention, and treatment efforts.

• Collaborate with external providers to manage patients with chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) by creating individualized, evidence-based care plans aimed at optimizing health outcomes.

• Oversee patients with substance use disorders and prescribe medications for opioid use disorders, such as suboxone, as well as medications for alcohol use disorder, including naltrexone (IM/PO) or acamprosate (this entails suboxone induction and outpatient alcohol detox for eligible patients, under the supervision of our Psychiatrist).

• Prescribe long-acting injectable medications for suitable patients.

• Provide health coaching, lifestyle counseling, and patient education to encourage self-management and improved health outcomes.

• Coordinate necessary referrals or home-based services with specialists and community partners.

• Participate in a rotating call schedule to address urgent clinical needs and foster healthier days at home.

• Offer clinical coverage and training in the Collaborative Care and Bridge Programs, including Behavioral Health Case Conference, to cover for colleagues on leave or vacation.

• Work in close partnership with Health Advocates, providing clinical support and guidance to assist them in coaching and empowering patients for effective self-management of their health, particularly in outpatient SUD treatment, LAI management, and mental health accompaniment.

• Collaborate closely with our Behavioral Health Clinicians to support clinical objectives and co-manage patient care.

• Refer, collaborate, and co-manage patients alongside our nursing and pharmacy teams.

• Engage with external providers and community organizations to ensure that care aligns with patient goals, as well as their social, home health, and DME needs (this notably includes external psychiatrists and care managers in community mental health).

• Participate in the Advanced BH Case Conference, where treatment plans will be refined in a multidisciplinary manner, addressing clinical documentation, quality gaps, treatment planning, transitions of care, and complex care planning.

• Ensure accurate and appropriate diagnosis, coding, and documentation of chronic conditions with evidence-based treatment plans, which encompass both physical and mental health conditions.

• Maintain timely and precise documentation in EMR and custom care platforms.

• Take responsibility for the quality of preventative care provided by assisting patients in closing HEDIS quality gaps.

• Respond promptly to clinical documentation inquiries.

• Quickly adapt to new tools, software, and systems—including AI-driven solutions—that enhance patient engagement, care coordination, and clinical decision-making.

• Comprehend the intersection of healthcare and technology, utilizing data-driven platforms, electronic medical records, and AI-enabled insights to independently resolve issues and improve patient outcomes and operational efficiency.

• Perform additional duties as assigned.


⛳️ Requirements

• Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of employment, along with a willingness to obtain additional licensure as requested.

• Active DEA registration or eligibility to obtain; X-waiver preferred but not mandatory.

• Active or eligible state-controlled substance license.

• Current Basic Life Support (BLS) certification.

• Proven strong clinical judgment and capability to make independent medical decisions.

• Proficiency in utilizing electronic medical record (EMR) systems.

• Over 5 years of experience working with high-risk populations facing serious mental illness and substance use disorder, along with co-occurring complex medical conditions.

• At least 2 years of experience in assessing, providing provisional diagnoses, planning, and managing behavioral patient care gained through clinical experience.

• Expertise with DSM5 diagnostic criteria.

• Highly skilled in trauma-informed care and culturally sensitive practices.

• Experience and comfort working within an interdisciplinary care team, specifically collaborating closely with community health workers and primary care providers.
