Nurse Practitioner – Flex Team

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Conduct thorough health evaluations.

• Oversee complex patients with an emphasis on minimizing hospital stays and enhancing health results.

• Coordinate and ensure delivery of quality, consistent, and cost-effective care.

• Assist complex kidney patients in navigating the healthcare landscape.

• Provide holistic and integrated care solutions.

• Operate both independently and in collaboration with the healthcare team.

• Manage patients remotely throughout their kidney health journey, including early kidney disease, dialysis transition, transplant care, and life-sustaining dialysis.

• Record and code care from a Medicare Advantage risk perspective and perform health risk assessments for high-risk or chronically ill patients.

• Offer continuous management of chronic conditions such as CHF, DM, cardiovascular disease, and hypertension.

⛳️ Requirements

• At least two (2) years of experience as a Nurse Practitioner (NP).

• Current, unrestricted NP license to practice in the state.

• Valid Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.

• Intermediate proficiency with computers.

• Skilled in MS Word, Excel, and Outlook.

• Access to a home office and high-speed internet.

• Willing to obtain up to 6 additional state licenses for remote patient management.

• Commitment to providing care to patients with complex, chronic conditions.

• Preferred: experience in Value-Based Care or Population Health.

• Preferred: experience in documenting and coding from a Medicare Advantage risk perspective.

• Preferred: experience conducting Health Risk Assessments on high-risk or chronically ill patients.

• Preferred: over 5 years of experience as a Nurse Practitioner specializing in adult/gerontology populations.

• Spanish-speaking is preferred in certain markets.

• Experience with the longitudinal management of chronic illnesses, including CHF, DM, cardiovascular disease, and hypertension.

• Must pass pre-employment substance abuse testing and background verification checks.
