This is a fully remote position, open to applicants in Colorado.
• Conducts clinical chart reviews to verify the accuracy of documentation by utilizing established clinical criteria for the diagnosis of medical conditions.
• Leverages clinical expertise to assess how provider documentation, laboratory results, diagnostic information, and treatment plans are reflected in coded data.
• Evaluates and interprets clinical data to detect gaps, inconsistencies, and opportunities for enhancement in clinical documentation, querying providers through a concurrent query process in accordance with ACDIS/AHIMA Guidelines for Compliant Query Writing.
• Engages with providers and other members of the healthcare team to clarify information or supply additional documentation as necessary.
• Collaborates directly with clinicians to elevate the quality and completeness of documentation through the query process and/or provider education, focusing on accurate disease processes and severity, complexity, and acuity to ensure precise medical coding.
• Works alongside team members as part of the clinical review process to ensure high accuracy and consistency in outcomes.
• Achieves accuracy and productivity metrics to support the team's attainment of established departmental objectives.
• Coordinates with DaVita leadership to address documentation issues and emerging trends.
• Actively engages in all scheduled department meetings.
• Delivers individual and group educational sessions for clinicians, medical coders, and other relevant staff.
• Collaborates with data analytics, coding, compliance, and education departments for onboarding, ongoing, and targeted education for all IKC APPs and IKC partner providers regarding documentation requirements for Medicare risk adjustment.
• Master's degree with a current active clinical license (APRN or PA) (required)
• Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure free of restrictions in the state of residence (required)
• CCDS-O or CDIP certification (required or must be obtained within six months of hire)
• CRC certification (required or must be obtained within one year of hire)
• Over 5 years of clinical experience as a Nurse Practitioner (adult-geriatric or family) or Physician Assistant (preferred)
• At least 3 years of experience in clinical documentation improvement, coding experience, or equivalent (required)
• Minimum of 1 year of experience collaborating with advanced practice providers and other clinical and non-clinical staff (required)
• Strong clinical foundation in evidence-based medicine, encompassing chronic disease management and prevention.
• Familiarity with ICD-10 coding and documentation requirements and risk adjustment.
• Understanding of compliant query writing and processes.
• Capability to conduct a thorough chart review of 20-30 medical records daily, with variations in workload based on daily tasks.
• Technical proficiency in using EMR systems.
• Competency in Microsoft applications (Excel, Outlook, Teams, PowerPoint, Word)
• Willingness to travel for on-site meetings 2-4 times annually.
