Nurse Practitioner Contractor – Multi-State

This is a fully remote position, open to applicants in Alabama, +5 more states.

📋 Description

• Offer call-based support to participants in Perry Health’s Remote Patient Monitoring (RPM) initiative.

• Provide prompt, compassionate care to members while evaluating their clinical skills.

• Analyze medical histories and data to assess member eligibility for the clinical program.

• Act as the main clinical contact for practice-wide escalations.

• Work in conjunction with the larger care team — including RNs, RDs, and Accountability Coaches — to uphold clinical objectives and SMART goal advancement.

• Comply with Perry Health's policies, procedures, and standard operating protocols to maintain high clinical quality and compliance.

⛳️ Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution; current certification as a Nurse Practitioner.

• A valid, unencumbered state nurse practitioner license is mandatory. Multi-state licensure is also required.

• At least 3+ years of experience in managing diabetes patients.

• Knowledge of continuous glucose monitors (CGMs) and behavior change theory is advantageous.

• Eager learner, willing to gain proficiency in necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to facilitate sustainable behavior change is a plus.

• Creative thinker with attention to detail, capable of working independently, possessing a positive attitude, and able to thrive in a fast-paced environment while collaborating with diverse groups at all levels of the organization.

• Spanish-speaking is highly preferred.
