This is a fully remote position, open to applicants in Arizona, +2 more states.
• Act as the main clinical contact for patients as they navigate their remote care journey.
• Direct clinical decision-making for conditions such as heart failure, type 2 diabetes, and hypertension by utilizing evidence-based guidelines.
• Oversee a group of patients receiving remote care and make timely clinical judgments.
• Initiate and adjust guideline-directed medical therapy while partnering with physicians, RNs, and care teams.
• Quickly address clinical escalations from remote patient monitoring, which includes abnormal vital signs, symptoms, and lab results.
• Deliver both synchronous and asynchronous care, which encompasses reviewing patient-generated health data, responding to alerts, secure messaging, and scheduled virtual appointments.
• Fulfill productivity and documentation requirements while ensuring safe, efficient, and high-quality care.
• Adapt to changing workflows, software enhancements, and operational procedures.
• Utilize a variety of technology platforms and provide insights to enhance clinical operations and technology.
• Work collaboratively across clinical and operational teams to enhance patient care, efficiency, and scalability.
• Contribute to the development of processes for a rapidly expanding digital health organization.
• Active Compact Multi-State RN license.
• Active NP licensure in California, Washington, or Arizona, in good standing at the time of hire.
• Over 5 years of experience in treating patients with chronic diseases, including CHF, hypertension, and type 2 diabetes.
• At least 3 years of cardiology experience with a focus on heart failure management and guideline-directed medical therapy.
• Active ANCC or AANP certification.
• Master’s Degree in Nursing as a Nurse Practitioner.
• Willingness to expand state licensure as Cadence enters new markets and partners.
• Home office setup with reliable Wi-Fi and strong upload and download speeds; a Wi-Fi speed test is required during interviews.
• Proven success in fast-paced environments with frequently changing priorities, workflows, and technology.
• Comfortable with receiving regular feedback and continuously adjusting processes.
• Highly organized with excellent time management skills and the ability to balance patient care with productivity expectations.
• Self-directed and accountable while working remotely with minimal supervision.
• Active clinical judgment when employing AI-assisted clinical tools such as Gemini or Open Evidence.
• Preferred experience in a CHF bridge clinic or T2D clinic, a startup environment, and remote patient monitoring technology.
• Availability Monday through Friday; preference for later weekday coverage, including until 11:00 PM Pacific Time.
