📋 Description

• Deliver direct patient care and support by developing, monitoring, and implementing treatment plans in line with laws, regulations, and protocols.

• Prescribe medications following established treatment guidelines.

• Request laboratory tests and interpret results, providing explanations to patients.

• Educate patients regarding medications, potential risks, benefits, and realistic outcome expectations.

• Engage in effective communication with patients and conduct necessary follow-ups.

• Refer patients with higher acuity needs to specialized care based on strict screening criteria to ensure quality treatment and optimal outcomes.

• Maintain accurate medical records and uphold high standards of clinical documentation.

• Offer medical care, patient education, and medication management in a compassionate and non-judgmental manner.

• Contact members identified through remote monitoring protocols and, if necessary, prescribe emergency medications or direct them to community healthcare services.


⛳️ Requirements

• Nurse Practitioner (NP) with an active, unrestricted license in 10 or more states, preferably including PA, CA, NY, TX, FL, MD, and VA. (If you do not fully meet this requirement but are close, we encourage you to apply).

• Board Certified.

• Preferred experience in treating conditions such as CHF, COPD, Diabetes, and Obesity.

• Proven experience working collaboratively with patients, clients, and other healthcare providers in an integrated setting.

• Strong organizational abilities and keen attention to detail.

• Prior experience in quality assurance and improvement initiatives is preferred.

• Familiarity with digital technologies and general computer skills.

• Capable of working independently with minimal supervision while being accountable for specific goals and objectives.

• Exhibits problem-solving abilities, leadership, conflict resolution, and team-building skills to foster a productive work environment and achieve goals.

• Displays a strong ability to adapt to a fast-paced, start-up atmosphere.

• Proficient in the use of EMR (electronic medical record) systems.
