This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct assessments of patient needs and support the creation of tailored treatment plans.

• Equip individuals with essential resources to help them achieve their personal objectives.

• Collaborate with social and medical agencies, external medical and psychiatric care providers, and community outreach programs to ensure patients receive necessary services.

• Complete ongoing education courses to uphold licensure and certification, remaining informed about the latest practices.

• Address social, psychological, and behavioral disorders in partnership with an interdisciplinary team.

• Oversee direct treatment management, including the evaluation and prescribing of psychotropic medications.

• Offer after-hours consultation for urgent client matters within the caseload.

• Attend mandatory staff meetings and case conferences to contribute professional insights into the treatment plan execution and assessment of relevant issues.

• Engage in medical team and program staff meetings as scheduled.

• Prescribe psychotropic medications and conduct regular evaluations of therapeutic responses and potential side effects.

• Inform patients about psychotropic medications and their side effects.

• Work collaboratively as part of a multidisciplinary team.

• Carry out other medical responsibilities as requested, within the scope of the position.

• Enhance knowledge and skills by participating in educational workshops and conferences.

• Join interdisciplinary meetings to ensure the highest quality patient care.

• Collaborate with physicians, other Advanced Practice Clinicians (APCs), and nurses to create personalized care plans for patients.

• Cultivate a supportive and compassionate atmosphere for patients and their families.

• Execute additional duties as assigned by the CEO.

• Partner with navigators, therapists, peer guides, and other treatment team members to coordinate patient care.

• Ensure timely and accurate completion of medical records, including assessments, treatment plans, progress notes, and outcome measures in the electronic medical record system.

• Examine or order laboratory or diagnostic tests for clients to gather information on their overall physical and mental health.

• Respond to calls and emails promptly (within 24 hours or sooner if clinically necessary).

• Provide crisis assessment and management for patients and be accessible to NorthStar Care on-call providers.

• Make independent decisions based on protocols developed using national guidelines for treatment interventions.

• Adhere to all procedures and protocols in accordance with NorthStar Care policies, as well as federal and state regulations.

• Actively engage in all departmental education, training, and in-services to ensure comprehension of new information or changes to established clinical policies and procedures.


⛳️ Requirements

• MINIMUM REQUIREMENT – Must hold appropriate professional credentials and current licensure in at least one state as a Nurse Practitioner. An NPI number is required or must be obtainable upon employment.

• EXPERIENCE – Prior experience in addiction preferred.

• PRODUCTIVITY TOOLS - Proficient in key productivity tools such as Microsoft O365 (PowerPoint, Excel, Word, etc.) or equivalent.

• VIDEO CONFERENCING – Familiarity with administering, optimizing, or licensing video conferencing tools like Microsoft Teams, Zoom, GoToMeeting, Webex, or equivalent.

• MESSAGING PLATFORMS – Familiarity with administering, optimizing, or licensing messaging platforms like Microsoft Teams, Webex Teams, Slack, or equivalent.

• ELECTRONIC MEDICAL RECORDS (EMR) – Knowledge of electronic medical records (EMR) systems and documentation requirements to assist clinicians, nurses, navigators, and peer guides with workflow and documentation best practices.

• ADDITIONAL – Strong verbal and written communication skills.

• Willingness to work fully remote.

• Availability for on-call duties.

• Strong organizational skills.
