This is a fully remote position, open to applicants in Arizona, +6 more states.
β’ Conduct synchronous telehealth consultations to evaluate patients with chronic illnesses such as hypertension, dyslipidemia, diabetes, obesity, and early-stage kidney disease.
β’ Order laboratory tests and other diagnostics as necessary to evaluate the status of chronic conditions.
β’ Provide treatment recommendations to the referring provider.
β’ Review and respond to remote patient monitoring (RPM) data, lab results, vital signs, and symptoms reported by patients.
β’ Prescribe and optimize medical therapies in accordance with clinical practices and evidence-based guidelines for patients failing to meet clinical outcome targets.
β’ Achieve clinical outcomes and key performance indicators (KPIs) for a designated panel of patients.
β’ Act as a liaison between patients and other entities within the healthcare system, including providers, clinics, and pharmacies.
β’ Foster the internal development of clinical programs within a fast-paced and dynamic team setting.
β’ Engage in program development and operational workflows during the growth and implementation phases.
β’ A minimum of 3+ years of experience treating patients with cardiovascular, cardiometabolic, cardiorenal diagnoses, diabetes, and/or obesity in an outpatient environment.
β’ Must reside in a nurse compact state and possess an active multistate RN license.
β’ Completion of an accredited graduate-level Advanced Practice Registered Nurse (APRN) program.
β’ Active licensure as an Advanced Practice Nurse in at least 5 states, with a commitment to obtain additional state licenses as required. Must hold a license in at least one of the following states at the time of hire: Arizona, Colorado, Georgia, Illinois, Indiana, Michigan, Ohio, South Carolina, or North Carolina.
β’ National certification as an APRN with a focus on Adult, Adult-Gerontological, or Family health.
β’ Current enrollment as a Medicare Part B provider.
β’ Previous experience in digital health and remote/virtual patient management.
β’ A strong commitment to enhancing patients' cardiovascular health through the application of clinical practice guidelines and evidence-based medicine.
β’ Interest in leveraging technology to enhance healthcare and an aptitude for quickly learning and utilizing new software and tools.
β’ Excellent customer service skills, with the capability to identify obstacles and devise solutions.
β’ Proven ability to think critically and make decisions effectively under pressure.
β’ Capacity to manage conflict resolution while remaining patient and adaptable in all situations.
