This is a fully remote position, open to applicants in Washington.
• Conduct thorough Comprehensive Health Evaluations to identify and manage comorbid conditions.
• Attend to patients’ medical, social, emotional, and financial requirements.
• Aim to decrease hospital admissions and enhance patient outcomes.
• Oversee the care of patients with CKD and ESKD.
• Concentrate on decelerating disease progression, minimizing costs, and preventing readmissions.
• Utilize prescriptive authority, medication management, and interpret diagnostic results.
• Work collaboratively with nephrologists and interdisciplinary teams.
• Lead the clinical team and synchronize care with medical treatment plans.
• Attend to both medical and psychosocial needs of patients.
• Strong commitment to caring for patients with intricate, chronic illnesses.
• Openness to adapt as the program develops.
• At least 2 years of experience as a Nurse Practitioner.
• Active, unrestricted NP license in the state of practice.
• DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Valid driver’s license and insurance in the state of practice.
• Proficient computer skills in MS Word, Excel, and Outlook.
• Strong clinical judgment and ability to make autonomous decisions.
• Expertise in managing complex comorbidities.
• Positive, solution-oriented mindset.
• Home office setup and high-speed internet access.
• Reliable transportation.
• Preferred: 5+ years of experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology.
• Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments.
• Proven experience in managing population health with an emphasis on reducing hospitalizations.
• Prior experience with Cerner or similar EHR systems.
