This is a fully remote position, open to applicants in Arizona, +2 more states.
β’ Perform thorough telehealth assessments that encompass patient history, physical examinations, and diagnostic evaluations.
β’ Create and oversee tailored care plans for individuals with chronic lung diseases.
β’ Lead respiratory therapists in delivering direct patient care.
β’ Collaborate with physical therapists and referring providers to guarantee coordinated and comprehensive patient care.
β’ Educate and counsel patients on pulmonary health, self-management strategies, and lifestyle changes.
β’ Ensure precise and timely documentation utilizing BreatheSuite's EMR and Coaching Dashboard.
β’ Valid certification as a Nurse Practitioner with an active license in CO, AZ, or UT.
β’ Active advanced practice nursing license in good standing.
β’ DEA license is preferred.
β’ At least 2 years of experience as a licensed Nurse Practitioner.
β’ Strong clinical competencies with experience in primary care, pulmonology, or chronic disease management.
β’ Previous experience supervising or managing other healthcare professionals.
β’ Proficiency with or willingness to learn telehealth platforms, EMR systems, and remote monitoring technologies.
β’ Access to a computer equipped with a webcam and microphone, along with a reliable internet connection.
β’ Minimum commitment of 8 hours per week; 16+ hours preferred with consistent weekly availability.
