Nurse Health Specialist, California RN License

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver triaging services for patients contacting the virtual care center.

• Oversee patient care and treatment in conjunction with the Physician and Nurse Practitioner/Physician Assistant.

• Work alongside the primary care physician, Extensivist, Nurse Practitioner/PA, and Case Manager to formulate care plans for members.

• Conduct outbound calls and virtual consultations to ensure patient follow-up.

• Perform daily assessments of vital signs for patients participating in the remote patient monitoring program.

• Facilitate disease management referrals.

• Analyze and interpret diagnostic tests to recognize and evaluate patients' clinical issues and healthcare requirements.

• Provide education to members on subjects such as disease processes, end-of-life considerations, medication adherence, and compliance.

⛳️ Requirements

• A minimum of 1 year of experience as a Registered Nurse (RN).

• A Bachelor's degree in Nursing is preferred.

• Current and unrestricted nursing license for California, Nevada, and North Carolina.

• CPR certification is required.

• Preferred experience in gerontology and adult care.

• Experience in palliative/hospice and complex care management is preferred.

• Experience in Home Health, including wound care, is preferred.

• Knowledge of Medicare Managed Care Plans is preferred.

• Bilingual skills are valued, with Spanish preferred.
