Nurse, Care Management

This is a fully remote position, open to applicants in Florida, +1 more state.

📋 Description

• Perform outreach to patients for assessments, care coordination, and resource provision.

• Develop, implement, and revise personalized care plans for patients involved in care management programs.

• Inform patients and their families about disease processes, preventive screenings, and health maintenance strategies.

• Ensure accurate and timely documentation of patient interactions, progress notes, and case updates.

• Organize interdisciplinary care team meetings and relay patient progress, challenges, and care requirements to healthcare providers, families, and team members.

• Evaluate physical and psychosocial health, identify care gaps, and advocate for essential services.

• Manage transitions of care and track progress to modify care plans as necessary.

• Work collaboratively with physicians, interdisciplinary teams, and community resources to guarantee smooth care delivery.

⛳️ Requirements

• LPN certification is required; RN certification is preferred.

• A Bachelor’s degree is preferred; however, four years of relevant clinical experience is mandatory.

• 2–3 years of nursing experience is required, including at least 1–2 years in care management, case management, utilization management, or care coordination.

• Proficiency in Microsoft Office and electronic medical record (EMR) systems is necessary.

• Familiarity with Salesforce is preferred.

• Outstanding written and verbal communication skills are essential.

• Strong organizational skills and effective time management abilities are required.

• Proficient in motivational interviewing and telephonic nursing techniques.

• Capable of working independently while also collaborating within an interdisciplinary team.

• Bilingual proficiency in English and Spanish is preferred.

• Availability to work extended and flexible hours, including weekends as needed.

• Must be able to lift up to 50 lbs.

• May involve 0–25% travel to off-site clinics, hospitals, or other healthcare facilities.
