Nurse Care Associate

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Engage in telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care initiatives.

• Analyze biometric alerts to identify readings outside of normal ranges; escalate clinical issues as per established protocols.

• Deliver patient education and coaching to enhance adherence to care plans and lifestyle modifications.

• Precisely document patient interactions in EMR/RPM systems, ensuring compliance with clinical and regulatory guidelines.

• Work collaboratively with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to coordinate patient care effectively.

• Assist in patient engagement efforts aimed at enhancing monitoring compliance and improving clinical outcomes.

⛳️ Requirements

• Valid LPN/LVN license in good standing.

• Over 5 years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Proficient in telephonic communication, patient education, and motivational interviewing techniques.

• Familiarity with EMR/EHR systems and comfort in a technology-driven care environment.

• Capacity to work independently within a structured remote clinical workflow.
