This is a fully remote position, open to applicants in United States.
• **Position Summary:** The Navigator offers case management services to patients registered at NorthStar Care. This individual acts as the primary point of contact for patients, fostering strong relationships to ensure they remain engaged in their medical care and treatment. Navigators are dedicated to eliminating obstacles to care by identifying essential resources, guiding patients through healthcare services and systems, and advocating for their treatment. They collaborate closely with the Care Team, which may consist of doctors, nurses, and other clinical personnel to enhance positive healthcare outcomes for patients.
• Responsibilities
• - Develops strong relationships and serves as a contact point for patients during their initial onboarding and orientation, along with ongoing monthly engagements to assist with navigating their NorthStar Care treatment.
• - Aids in registering patients for the patient portal and facilitates the completion of patient consent forms for treatment.
• - Organizes medical appointments in coordination with the Care Team.
• - Communicates effectively with Care Team members, including primary care physicians, nursing staff, peer guides, and other healthcare providers, to enhance patient care.
• - Orders treatment supplies for home delivery for patients.
• - Identifies resources to help patients overcome barriers that may hinder their treatment.
• - Stays informed about current services and educational resources provided by NorthStar Care.
• - Upholds strict confidentiality in compliance with NorthStar Care policies.
• - Meets with the Care Team (including but not limited to the Care Coordinator and primary care provider) to discuss patient care issues and requirements during treatment.
• - Maintains thorough documentation of all patient interactions and fulfills reporting obligations per organizational standards.
• - Tracks patient information, schedules, files, faxes, and forms with confidentiality.
• - Monitors patient attendance at medical appointments and navigation sessions, initiating outreach and procedures for missed appointments as necessary.
• **PERSONAL CHARACTERISTICS AND SKILLS:**
• - Demonstrates passion, trustworthiness, and empathy when engaging with patients.
• - Capable of building relationships with diverse individuals, including patients, organizational members, and healthcare providers.
• - Possesses strong communication and interpersonal skills, able to convey information concisely to patients and Care Team members.
• - Highly organized with respect to confidential patient materials and appointment management.
• - Flexible and adaptable to meet the evolving needs of patients and healthcare providers.
• - Exhibits a genuine interest in working with patients who are actively seeking treatment.
• **EDUCATION AND EXPERIENCE:**
• - Minimum of a high school diploma or some college education.
• - Strong understanding of cultural competence concerning the target population.
• - Bilingual (English/Spanish) preferred but not mandatory.
• - Computer literacy is advantageous.
• - Familiarity with treatment-related issues.
• **PHYSICAL REQUIREMENTS:**
• - Physical demands typical of home office work.
• - Reliable high-speed internet connection at home.
• - Availability for some evening and weekend work required.
