Mental Health Physician Assistant/NP

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct follow-up telehealth visits for the management of psychiatric medications in elderly patients.

• Monitor treatment efficacy and modify medications while considering age-related risks, polypharmacy, and existing medical conditions.

• Execute cognitive screenings and assessments using tools such as MoCA, MMSE, or equivalent.

• Create initial care coordination and behavioral support strategies.

• Collaborate closely with nursing staff to ensure ongoing patient monitoring and support.

• Communicate with the supervising psychiatrist as required.

• Keep precise and timely documentation within the electronic medical record (EMR).

⛳️ Requirements

• Must be licensed and certified as a Physician Assistant or PMHNP in New York (required).

• Preferably have experience in geriatric psychiatry, long-term care, or dementia care.

• Familiarity with cognitive assessment tools and strategies for behavioral management is essential.

• Must be comfortable providing care through telehealth platforms.

• Strong clinical judgment regarding polypharmacy, fall risk, and medication safety in older adults is necessary.

• Ability to work effectively within a team-based care framework.
