This is a fully remote position, open to applicants in New York.
β’ Conduct follow-up telehealth visits for the management of psychiatric medications in elderly patients.
β’ Monitor treatment efficacy and modify medications while considering age-related risks, polypharmacy, and existing medical conditions.
β’ Execute cognitive screenings and assessments using tools such as MoCA, MMSE, or equivalent.
β’ Create initial care coordination and behavioral support strategies.
β’ Collaborate closely with nursing staff to ensure ongoing patient monitoring and support.
β’ Communicate with the supervising psychiatrist as required.
β’ Keep precise and timely documentation within the electronic medical record (EMR).
β’ Must be licensed and certified as a Physician Assistant or PMHNP in New York (required).
β’ Preferably have experience in geriatric psychiatry, long-term care, or dementia care.
β’ Familiarity with cognitive assessment tools and strategies for behavioral management is essential.
β’ Must be comfortable providing care through telehealth platforms.
β’ Strong clinical judgment regarding polypharmacy, fall risk, and medication safety in older adults is necessary.
β’ Ability to work effectively within a team-based care framework.
