This is a fully remote position, open to applicants in United States.
• Oversee the design, execution, and management of housing initiatives aimed at assisting Medicaid members experiencing homelessness.
• Serve as a subject matter expert, providing guidance to internal teams regarding housing-related issues.
• Work collaboratively with interdisciplinary teams to ensure holistic and integrated service delivery.
• Establish and sustain partnerships with shelters, housing providers, property developers, and local government entities.
• Tailor housing solutions to address the unique needs, accessibility, and sustainability requirements of members.
• Assess and evaluate program effectiveness, implementing data-driven modifications as necessary.
• Provide training and support to case management teams regarding housing resources and transition strategies.
• Connect with community organizations and stakeholders to tackle housing challenges and explore collaboration opportunities.
• A Bachelor's degree in Social Work, Human Services, Public Health, Urban Planning, or a related discipline; relevant experience may be accepted in lieu of a degree.
• At least 2 years of experience in case management, housing coordination, or a comparable position.
• Familiarity with Medicaid regulations and eligibility criteria.
• Experience engaging with diverse populations, including individuals with disabilities and low-income families.
• Strong communication, organizational, and interpersonal skills.
• Capability to work both independently and collaboratively within a dynamic team environment.
• A valid driver's license and dependable transportation for travel to client meetings and housing locations.
• Preferred: Licensed Social Worker (LSW) certification.
• Preferred: Knowledge of local housing resources, services, and community organizations.
• Preferred: Bilingual or multilingual skills.
• Preferred: Experience in developing or managing affordable housing projects.
• Preferred: Understanding of Medicaid programs and Social Determinants of Health (SDoH).
